All staff, including contractors, service providers and volunteers of Queensland Representative School Sport programs, must comply with relevant privacy and information management legislation, department procedures and guidance.
Consent form
A consent form is required in order to obtain a written record of permission for the use and disclosure of you and your child’s personal information.
This information will be used and disclosed by authorised departmental employees for the purposes outlined on the consent form. Examples of personal information which may be used and disclosed (subject to consent) include student names, school, school year level, date of birth, age, image/photograph or voice/video recording.
If you wish to access or correct any of the personal information on the consent form, or discuss how the information is managed, please contact Queensland Representative School Sport in writing at

The consent form is for representative school sport events run by Queensland Representative School Sport and Regional School Sport offices.
Third party providers
Personal information may also be disclosed to third parties as authorised on the consent form, or where authorised or required by law. It is important that you read the relevant privacy policies of these third party organisations prior to making a decision about consent.
As we continue to improve and refine our use of technology to drive contemporary representative school sport, our thirty party provider lists may be edited. Please check the lists prior to each event to ensure that you have the most up to date information and can make an informed decision about consent.
